Use this form to trigger a Guardian confirmation process. This will notify team members as required, automatically send a pre-formatted welcome/congratulations email along with a link to fill in their required trip paperwork via an online form. If U.S. mail is required, this will trigger a notification to that individual to send off all trip papework via U.S. mail.
